The City of Stonnington has an exciting opportunity for an experienced Assessment Officer to join our Aged Services Team until 30th June 2019 with the possibility for extension.

We are seeking candidates for an immediate start and advertising will close once a suitable applicant has been found. This position is full time for 5 days a week, however, flexible work days may be considered.

You will be responsible for completing Living at Home assessments and ongoing Support Plan reviews of residents within the aged and disability group target in the community. As an Assessment Officer, you will also be responsible for the approval and referral of services under the Commonwealth Home Care Programme, Home and Community Care Programme for Younger People and other community and Council Services.

You will have a tertiary qualification in Health, Allied Health or Social Science and substantial experience in assessments in Local Government or the Community sector. Additionally, you will have detailed knowledge and understanding of the principles of Aged and Disability Services for the community and substantial experience in providing good customer service to a wide range of people from diverse backgrounds and life experiences. Experience in conducting Living at Home assessments in the community and use of the My Aged Care Portal are essential.
